My 2006 MDX just starting making this weird engine noise that only occurs at idle coming from the left side of the engine. Current mileage 125,000 miles. Timing belt, water pump, alternator, serpentine belt, and tensioners all replaced in 2015 @107,000 miles. Just purchased vehicle as of August 2016 with 114,000 miles. Car was meticulously maintained by original owner following "severe" conditions maintenance schedule with 20 service records from Acura dealership. Car runs outstanding in about every way besides this one annoyance. Just want thoughts and opinions about what the noise could be and if I should have concern. And I would love any maintenance tips to follow to keep her running past the 200k mark. Valves have never been adjusted, would it be a good idea to adjust soon or wait until I start seeing major performance problems? Already did 3x drain n fill on transmission with Valvoline max life and bottle of lube guard red and plan to change fluid at least once a year to keep transmission in it's perfect condition. Also change the oil and filter every 3,750 miles using Mobil 1 full synthetic. I really really really love this car more than any other, and want her to keep her til the wheels fall off LOL! Take a look at the thread right above or below this one, regarding power steering pump noise. Hard to know if that's the same noise, but I consider it an easy preventive effort for around $5. The orange o-ring might be an aftermarket part offered at dealers to resolve the shrinking o-ring problem in the pump's inlet joint.

I would do this first as I imagine if the noise continued even after o-ring replacement, whatever it is, will be much more than a $5 repair.

I'd sincerely recommend buying a cheap ($5?) mechanic's stethoscope from Harbor Freight, and you'll be able to find the source of the noise in no time. Just put the "probe" on various parts of the engine / components, and when you hit the right one, it'll be obvious where the noise is coming from.

That said, I doubt it's the power steering O-ring (based just on the noise it's making), though it's a great maintenance item for the effort / cost.

The noise makes me think there may be a motor mount making metal-to-metal contact. You could always look for excessive motor movement when the car is moved from N-D-R (or the tell-tale "clunk" that signals a motor mount going south). There's a whole lot going on at that end of the motor though... everything that's turned by the timing and accessory belts, for example, plus all the valve train components. Just hope it's something simple and external (it usually is).
