So dealer R/R'd my alternator and I immediately started hearing a squeal above 1500 rpm on my 05 MDX. It lasted a couple days and I took it back and they said my belt tensioner is bad $523. How could it have gone bad at alt change? I think they are full of crap, its more likely a bad alternator.

I am a retired tech. There were a few times where the tensioner started making noise after a belt remove and replace when we did timing belts. Not often, but it is POSSIBLE. Price sounds out of line. Labor to replace the tensioner should only be about an hour.
