I decided to double check the manual based on your comments plus just my belief that this thing should up-shift by itself even in S. According to the manual:

S Position: .....This position is similar to D, except only gears from first to fifth are selected....The S position keeps the transmission from cycling between fourth, fifth, and sixth gears in stop-and-go driving....once you begin to use the paddle shifters in S, the transmission will no longer upshft or downshift automatically.

I'm not sure this is super clear...it says it locks out sixth in the first part but then says it locks out 4th-6th around town. I suppose it should sense the higher speeds once you leave "town" and then shift up to 4th/5th...and I wonder if that's where I have a problem since it doesn't seem to go above 3rd even getting up into the 50mph range. Even if it's working properly I don't have much faith my wife would remember to go back to D on the highway and according to this it would stay in 5th gear. Not a big deal but obviously far from ideal.

We have a brand new 2012 MDX bought last week. I notice the engine seems to lug when you begin to accelerate at low speeds. You can feel slight vibration in gas pedal and steering wheel. Probably same issue as others have noted in which all makes and models of new cars try to stay in higher gear to improve gas mileage. Maybe I will become a Formula 1 driver and use the paddle shifters more often.

The X transmission adapts to your driving style. You may want to wait a couple of weeks until the transmission gets used to your shifting patterns.
